If you need to file a grievance that does not require completing a CAPTCHA or placing a telephone call, you can (1) complete the attached form and mail it to the Grievance Department or email it to Ms. Nash; (2) create a document that includes the information requested on the attached form and email it to Ms. Nash, or (3) complete the attached form, except for the Description section, and in that section, write 'See Attached' and include a separate document detailing your grievance.
