April 2, 2026: From Recovery, a Request to Reopen and Reassign

The denial came the day of surgery. The next day, from recovery, I asked the supervisor to reopen the file and answer the questions the adjuster had not.

What it is

The day after the April 1 denial, from post-operative recovery, an email to Robert Angotta, the Gallagher Bassett supervisor, with a letter attached asking that the file be reopened and reassigned. Copied to Ms. Boscoe and to GIRMA’s claims and helpline addresses.

Why it matters

This is the third written request and the first to the supervisor level. It restates the two unanswered asks from March 13 and adds a third: identify the policy provisions the denial relied on. Most of the letter concerns the underlying claim and is outside this record; the accommodation sections are transcribed below.

Mr. Angotta,

I am writing to formally request that Claim No. [redacted] be reopened, reassigned to a new claims handler, and evaluated against the claims actually filed in my February 13, 2026 Third-Party Claimant Notice.

The attached letter documents the basis for this request in detail, including:

1. The April 1 denial evaluated “administrative tasks” — not the [redacted; outside this record] claims actually filed
2. The denial cites policy provisions but identifies none
3. Outstanding ADA accommodation requests from March 13 remain unanswered
4. The requested supervisor call has never occurred

Ms. Boscoe,

I have received your letter dated April 1, 2026. I am writing from post-operative recovery following the emergency surgery I notified you about in prior correspondence.

Your letter has fundamental problems that I need to bring to the attention of your supervisor and GIRMA leadership.

2. Your letter invokes policy provisions but identifies none.

Your April 1 letter states that Gallagher Bassett “compared those facts to policy provisions, and concluded that your claims are not covered under the applicable policy.” It does not identify which policy provisions were applied, which exclusions were invoked, or how the [redacted] fall outside coverage. A denial that cites policy provisions without identifying them is not a completed coverage determination. It is a conclusion without analysis.

Please identify the specific policy provisions that exclude the claims identified in the February 13 notice.

4. You had notice of disability, medical treatment, and accommodation needs, and did not answer the accommodation questions before closing the file.

I wrote on March 13 and followed up on March 16. I explained my hand limitations, the confusion created by the claim process, and my need for assistance navigating it. I offered to provide formal ADA documentation. No response addressed those requests. The next communication from your office was the April 1 denial.

My originally scheduled procedure had to be rescheduled twice because of two emergency surgeries at Mayo Clinic. I communicated these developments to your office. You had notice of my medical treatment, disability-related communication needs, and the April 1 surgery date before the file was closed. I now have a stent that requires a second surgery within three weeks to remove.

You did not provide an ADA accommodation contact. You did not return a phone call. You did not respond to the offer of ADA documentation. Every request made on March 13 remains outstanding.

I am requesting that GIRMA:

1. Reopen the file and reassign it to a new claims handler.

3. Provide the ADA accommodation contact information requested on March 13 and still outstanding.
4. Schedule the supervisor call requested on March 13 and still outstanding.

Preserve the complete Gallagher Bassett / GIRMA claims file for Claim No. [redacted], including internal notes, call logs, claim diary entries, emails, attachments, routing records, download/access logs, and all communications concerning coverage evaluation, accommodation requests, reassignment, and closure.
